Shared Lives Dementia Service Survey
We Want to Hear from You about our new Shared Lives Dementia Service
Shared Lives is a unique, person-centred service that supports adults with a learning disability to live within our communities, sharing their life with a regular family. From 1 September 2025, Shared Lives Stoke will also be supporting adults with dementia in a day-support capacity. Based on their assessed need, they will be supported up to four hours a day to access the community or to use the Shared Lives carer's home as a base.
We would love to hear from a range of groups about what this support might look like as we are keen to co-produce the service. These groups include:
- Any person with dementia
- Unpaid carer of people with dementia
- Staff members providing direct care to people with dementia
- Staff members providing indirect care to people with dementia (e.g. social workers, wellbeing assessors, wellbeing coordinators)
- Shared Lives carers
