Use this form to apply for an apprentice disregard.
This form should be completed by the employer on behalf of the apprentice.
For the apprentice to be eligible for the disregard they must meet the following definition:
Either
1) Is employed to learn a trade, profession, business, office, employment or vocation
2) Is undertaking a programme of training leading to a qualification accredited by the Qualifications and Curriculum Authority (was National Council for Vocational Qualifications)
AND
3) is receiving a salary or allowance of less than £195 per week
