Tender and Quotation Opportunities Please find details of your selected tender and quotation opportunity below : TitleCarers Survey - Printing, Mailing and Data EntryContract Reference NumberPHASCS/2018/78InformationStoke-on-Trent City Council will be conducting the Survey of Adult Carers in January 2019. This is part of the Department of Health requirements in line with the National Minimum Data Set and Adult Social Care Outcomes Framework. The guidance for completing the survey sets out a minimum sample size and required response level. https://digital.nhs.uk/data-and-information/data-collections-and-data-sets/data-collections/social-care-user-surveys/social-care-carers-survey-2018-19 We anticipate that we will need to send out 1,100 surveys to meet the 5% confidence level. We need a total return of 285 surveys to meet the minimum requirements. There is one survey, defined by the guidance. The example/ model questionnaire can be found using the link above, however, we will provide a version to use (you can adjust the formatting of these to fit in with your surveying/ printing software as necessary within the restrictions set out in the guidance) but the final versions will require sign off before commencing the exercise. The survey will be 22 sides (11 pages), back to back, black and white. We will require a quote incorporating the following: • Printing and mailing of postal questionnaires including black and white 2-sided letter and reply paid envelopes for return. Questionnaires must carry a sequential reference number so we can correlate responses to the individual demographic / client information. On submitting the answers to each question, we have to include (for each person responding) their age, gender, ethnicity, service breakdown etc. which we will have to match when we receive the results from you. We have to include this information on a person-level basis with our response to the Department of Health. • Single mailing to all in the sample week commencing 7th January and one reminder mailing, including another 22-side questionnaire and 2-sided reminder letter, to non-respondents after three weeks. • Checking and data entry of returned questionnaires - all question numbers in the data capture system and in turn spread sheet of results sent to us at the end, must remain the same as those in the questionnaires. Please do not change question numbers or ordering in the return file. • Providing a completed spread sheet of individual-level results to the Council in the Excel Spreadsheet template provided. The survey must be completed and the results returned to us by 25th February 2019.
