InformationStoke-on-Trent City Council will be conducting the Adult Social Care User Survey in March-April 2018. This is part of the Department of Health requirements in line with the National Minimum Data Set and Adult Social Care Outcomes Framework. The guidance for completing the survey sets out a minimum sample size and required response level and can be found at http://content.digital.nhs.uk/article/7879/User-survey-guidance---2017-18 Using the 2017 client base we anticipate that we will need to send out 1,600 surveys to meet the 5% confidence level. We need a total return of 350 surveys to meet the minimum requirements. There are eight different types of survey defined by the guidance, however, due to very little variation between the “community” and “residential” versions we use three surveys only. We use one survey for learning disability clients and two (standard print and large print) for all other clients. The example/ model questionnaires can be found at the above link - under “Questionnaires”, see 1a, 1b and 2a. However, we will provide the three versions we will use. You may adjust the formatting of these to fit in with your surveying/ printing software as necessary but we are not permitted to reduce the number of pages. The final versions will require sign off before commencing the exercise. Questionnaires must carry a sequential reference number so we can correlate responses to the demographic information/ client information we hold. •Clients with Learning Disabilities, required sample: 306 sent out, require 67 responses Standard print: 29 sides (15 pages), back to back, colour - All other clients - required sample: 1,294 sent out (1,266 standard print, 28 large print), require 283 responses a) Standard print: 16 sides (8 pages), back to back, black and white b) Large print: 24 sides (12 pages), back to back, black and white We will require a quote for the following: 1. Printing and mailing of postal questionnaires, to go out from 19th March, for three different sets of clients as instructed, including a four sided cover letter with pictures for clients with learning disabilities, a four sided large print cover letter and a two sided standard print cover letter. A reply paid envelope for return. 2. Checking and data entry of returned questionnaires into the spreadsheet provided using the specified coding (e.g. 1 for Yes; 2 for No; -9 for Not Answered). The question numbering and order must remain the same as those in the questionnaires. The data must be matched against the appropriate respondent using the sequential number. There are no open-ended questions in the survey. 3. Providing the completed individual-level spreadsheet of results to the Council, in Excel spreadsheet or CSV file. Please give the quote broken down over the three items above based on an initial postal of 1,600 and a final return of 350 responses. Please note that these numbers may vary slightly depending on the initial responses. The survey must be completed and the results returned to us by 4th May 2018.
