Fuse Outreach was built from the belief that every adult, no matter their circumstances, deserves dignity, safety, and a genuine chance to rebuild their life. Our team came together after working across homelessness services, addiction support, mental health care, and community outreach. We saw the gaps — the people falling through them — and we decided to create something different.
We’re a small team by design we are nurse led, and that’s our strength. It means we know every person we support. It means we show up consistently. It means we don’t hide behind systems or bureaucracy — we meet people face to face, on their terms, in their environment, whether that’s on the street, in temporary accommodation, or in their own home.
Our experience spans complex needs, drug and alcohol support, crisis intervention, safeguarding, trauma informed care, and specialist cleaning for high risk or challenging environments. We’ve worked with adults who have been excluded, overlooked, or written off — and we’ve helped them stabilise, reconnect, and move forward.
Fuse Outreach is built on action. We don’t wait for people to come to us. We go out, we engage, we listen, and we support. From emergency welfare checks to structured outreach visits, from harm reduction work to deep cleans that restore safety and dignity, we’re there for the moments that matter.
