Hi I’m Jasmin. I am here to provide you care and support with most of your daily activities. I have worked in different health settings over the years including mental health settings, dementia care, supporting young people in their own homes, and care homes.

The support that I offer is tailored according to your personal needs as I understand that we are all unique and our experiences differ from person to person. My support is not limited to one particular area of work as I can work with people with various challenges. Here is the list of some of the support that I provide:

Companionship

Support to attend outdoor leisure activities, using public transport

Support with engaging in indoor leisure activities for example board games, art and crafts, crossword puzzles

Support and assistance with household chores

Prompting with medication

Picking up prescriptions

Prompting and support to engage in personal care activities

Support to do activity timetables

Support to look for leisure activities in the community for example looking for gyms , crafts groups, volunteering opportunities

Support with booking and attending appointments

Support with reading mail, completing forms

These are just a few examples of what I can offer, please do no hesitate get in contact to discuss your needs and how I can help. The areas I cover are Sneyd Green, Milton and Birches Head, however there is some flexibility with this.

My working days are Monday to Friday between 7 am and 5 pm.

Whilst I do not work during weekends, arrangements can be made for one off activities or short-term work at weekends.

Please note that support with accessing outdoor activities is currently via public transport .
