Contracts ID Badge Application

Use this form to:

  • Apply for a contracts ID badge to transport vulnerable children and adults
  • Renew a contracts ID badge
  • Request a replacement ID badge (for lost contract ID badges or change in company)

Requirements:

  • Photo (this needs to be an image of your head down to shoulders against a clear background)
  • Proof of ID (driver’s licence or passport)
  • DBS certificate subscribed to the Update Service (this will be checked on-line if there are any changes a new one may be required)
  • Safeguarding training certificate (we will check if its acceptable) or the date of the safeguarding training if it was with Stoke City Council
  • Proof of address (household bill dated within the last 3 months or current year council tax bill)
  • Front of taxi and private hire licence or CPC card (this is not applicable for passenger assistants)

If you have a disclosure on your DBS, we will need to interview you and you will need to be cleared by our Safer Employment Panel. If your training is not acceptable you may HAVE TO ATTEND MORE TRAINING

You need to have a good understanding and be able to communicate clearly in English to work on contracts.
