This claim form is for parents/carers/young people that have been authorised by the Local Authority to receive a Personal Travel Budget (PTB). A personal travel budget is a contribution towards the cost of travel to support families to make their own arrangements for a child or young person’s journey between home and school. Please note: Claim forms must be submitted after the last journey made of the month. Claims will be processed within 15 working days. Claims must be submitted within 3 months of the last journey. If you do not receive payment within 15 working days, please email TransportRequests@stoke.gov.uk
