Fair processing notices - Community wellbeing enablement team

How we use your data

When you use the community wellbeing enablement service or Stay At Home Service, we collect information about:

  • your name
  • address
  • date of birth
  • your GP surgery
  • medical conditions and medication
  • people who provide care for you
  • key safe codes
  • the issues you face in carrying out your daily living activities
  • goals you would like to achieve to be as independent as possible
  • what equipment or adaptations we are able to provide for you

We only keep this data for the time specified in our retention schedule; this is for eight years after we finish working with you.

We use this data to assess to provide short term enablement-focused support and longer term support to promote independence and encourage individuals to regain and maintain their optimum ability during which time a full assessment will be completed. 

We also use this data to enable us to see what aids and adaptations you might require to meet your needs, to replace the equipment or adaptation if necessary in the future, and to make sure we review your support where needed.  

We are able to do this because the Care Act 2014 and the Health and Social Care Act 2008 says we must do it if you request an assessment from us.

We are required by Government guidance to hold and use some of your information for statistical purposes so we know the number of people using our services. This data is used by the council’s performance team and is made completely anonymous so you cannot be identified from it.

We may share data with our internal audit team to evaluate the effectiveness of the organisation’s risk management, control and governance processes.  We may also share your data with the council's fraud team to help to prevent and detect fraud.
