In many cases the most appropriate response to a specific point will be to provide copies of existing club documentation, such as your constitution, and refer to this in your response. There are a number of documents that need to be evidenced (please check the webpage before starting to complete this form). You need to return your completed application pack to us no later than Friday 13 August for Ball Green Playing Fields. If it is not received on or before this date, it will be assumed that your organisation wishes to withdraw from the self-management process.
