Over the Christmas school holidays Stoke-on-Trent City Council will continue to enable parents’ access to food vouchers for children across the city. There are two forms on this page so please ensure that the correct form is completed for your child. Form 1 is for children with an Education and Health Care Plan and form 2 is for pre-school children.
**Please do not fill in this form if your child is at school in the city. Schools will issue a voucher for eligible children. Please contact the school for help if you have not received a voucher by the start of the school holidays for the Christmas period.**
1) Food vouchers for children with an Education and Health Care Plan
If you and your family live in Stoke-on-Trent, your child has an Education and Health Care Plan and is educated at home or in a school outside of the city, you may be entitled to a £35 voucher for food for the School Holidays for the Christmas period 2021. This is made up of £15 per week’s holiday and £5 uplift for Christmas Day.
If you are eligible you will be sent a voucher by email and/or text so that you can download a voucher for the supermarket of your choice.

2) Food vouchers for pre-school children
Stoke-on-Trent City Council is providing a £35 voucher for food for the School Holidays for the Christmas period 2021. This is made up of £15 per week’s holiday and £5 uplift for Christmas Day for families with children who live in the city and attend a funded two year old place; or are eligible for Early Years Premium.
If your child attends a school nursery, vouchers will be provided directly from the school nursery. If your child is not at nursery at the moment or attends a private nursery you can apply by completing the form below.
****IMPORTANT – Any form received after 4pm on 23rd December 2021 will not be processed until 4th January 2022****
