The Leaving Care Team

What do we do?

We work with young people between the ages of 15 and 21 (or up until the age of 24 if they are full time education) who have been ‘looked after’ through the City Council and are now ready to move on. Being ‘looked after’ means that you either live with foster carers or in a children’s home. Before they leave, we start working with these young people to help them plan for their future.

How can we help you?

If you are a young person being looked after, we will normally begin working with you, your carers and Area Social Worker, after your 15th birthday. But we have more direct contact after your 16th birthday. It is around this time that we give you a Personal Adviser and work with you to write a ‘pathway plan’. This plan includes where you want to live and whether you want to stay in full-time education or do any training.

We can also help pay your living costs when you leave care. This includes money for rent, furniture, equipment for the home and clothes. As well as helping with these practical things, we also want to make sure you are healthy and happy. We will help you register for health care and ask if you want to receive any counselling.

Things you might want to ask us about...

The practical stuff
- how to look after your money, where you can live, help with jobs in the home like cooking, washing and ironing, what education, training or paid work you could do.

The personal stuff- drugs and alcohol, pregnancy, advice on safe sex,  contacting your family.
