What is the register?
The register is a database that holds information about disabled children and young people who live in Stoke-on-Trent.

If you are the parent of - or have parental responsibility for a disabled child - you can get their details added to the register by completing our simple registration form.

Why do we have a register?
The Children Act 1989 requires us to keep a register. The register helps us to plan and develop services for children with disabilities.

It also helps us involve and inform families and carers when planning and developing new services.

The information familes provide tells us whether or not we are providing the best and most useful services for children in the city. This helps us to identify gaps in services and make future improvements which may benefit your family.

Who has access to information from the register?
General information may be shared with partner agencies to help us jointly plan services.

Information on individual children will not be shared. However you are entitled to see information we hold about your own child.

Do you have to register your child?
No, the register is voluntary. Your child does not have to be on the register to receive services and support from health or social care.

How to register your child
Please follow the link below to download a registration form. Alternatively you can fill in our online form by following the link to the righthand side of this page.

We will tell you when your child's details have been added to the register.

You can request to have your child's details removed at anytime.
