Rehabilitation
What is rehabilitation?
We are a team of health and social care professionals who work with people who have lost, or are at risk of losing, their
independence. For example, people who have just come out of hospital and need a short period of support to help them fully
recover. We call this support rehabilitation.
We work with you to put together a plan of what care and support you will receive, to increase what you are able to do. We will set goals which you can work towards achieving.
How can we help?
We provide a planned programme of rehabilitation, which you could receive in a residential setting, in your own home
or through daytime activities. Where you receive rehabilitation will depend on your situation, but we aim to do provide it
in a setting which increases your independence. So if you are able to manage at home, we prefer to arrange rehabilitation
in your home or through daytime activities, rather than in a residential setting.
We will normally provide rehabilitation for up to six weeks.
Who can we help?
You need to be over 18, live in Stoke-on-Trent and need this type of short-term support. Many older people will benefit,
particularly those coming out of hospital who need help to recover. Or those who are at risk of being admitted to hospital
or of being permanently disabled.
Unfortunately we can't help everyone, so we have guidelines to help us decide who can use our services. The Government set these guidelines to make sure that everyone in the country has the same chance of getting help. To read more about these guidelines, click here.
How do you ask for this service?
Usually, another professional would refer you to us, like a social worker, occupational therapist, district nurse or your
GP.
But you can also contact us yourself. You can contact by ringing 01782-234050 or 01782-234235 where you can speak to a Contact
Centre Advisor.
What happens when you contact us?
We will ask you some questions to get some basic information, like your name, address and age. This information will be passed
onto one of our duty officers who will contact you within 48 hours to find out more about what help you need.
After this, the duty officer will pass the information onto a social worker or assessor who is specially trained to help someone
in your situation. They will need to carry out what's called an 'assessment' within 28 days. This usually means they will
come and visit you at home and talk to you more about what help you need. After that, the support you need should be set up
within 28 days.
Is there a charge?
The support is free for the length of your programme, which could be up to 6 weeks.
Other organisations that can help
Health professionals like Occupational Therapists or Physiotherapists can provide similar help. You need
