Transition - help and advice for parents and carers of children and young people with additional needs or disabilities.

Transition means different things to different people, but it can mean change in a person or families life. Going through transition or “change” can be difficult and can occur at any time in life. Change can be even more challenging if the person is a child or young person and has additional needs or a disability.

These pages are for parents and carers of children and young people with additional needs or disabilities. We aim to provide you with the answers to the questions you have, and to support children and young people in their transition to adulthood.

The following pages contain frequently asked questions submitted by parents and carers regarding issues around education, health, housing, social care and transport. The answers to the questions have been given by professionals familiar with the subject.
